Beyond this PTCAS application, we require TWO APPLICATIONS: 

  1. The CSU Mentor Graduate Online Admissions Application should be available online beginning 10/1/2011 at www.csus.edu/gradstudies/ and IS REQUIRED for application consideration by January 16, 2012.  This application is the one that requires a $55 fee.
  2. The Department of Physical Therapy Supplemental Application should be available online on or before 10/3/2011 and is due 1/16/2012.  This application is REQUIRED for application consideration, and contains a unique section that requires that the courses you enter for Anatomy, Physiology, Exercise Physiology and Kinesiology have been pre-approved by our program as meeting our pre-requisite standards.  If your course is not already on the approved list (see web site), then you must apply to have it approved.  The last day to submit the forms for this approval (called "Request for Course Substitution forms") is NOVEMBER 30, 2011.   

Applicants must complete both of the above applications in order to be considered for admission.  Please be aware that the application to the CSU Mentor Graduate Online Admissions is a separate process than the application to the Department of Physical Therapy.

Applicants must have completed AT LEAST 10 of the 12 prerequisite courses listed above by the 1/16/12 deadline.  This means a maximum of two outstanding pre-requisites are allowed for completion prior to the start of the program in August of 2012.

Additional information about program’s reference requirements: Three letters of recommendation on the provided forms need to be completed. Two of the forms need to be completed by  a physical therapist licensed in the U.S. or Canada,  who has worked with the applicant in a physical therapy setting. The supervision needs to have occurred when the applicant was either a volunteer or an employee at the clinic. The third letter of recommendation must be provided by a college level professor (either major or non-major) who has been an instructor of the applicant.

Additional information about program’s PT hours requirement

Applicants must have accumulated a minimum of 100 hours of volunteer OR paid experience under the supervision of a U.S. or Canadian licensed PT.  Hours must be completed in at least TWO different physical therapy setting types. 

  • Examples of setting types include: inpatient, outpatient, home, skilled nursing facilities (SNF). 
  • Each setting type should have at least twenty-five (25) hours of exposure.  For example, 75 hours in an outpatient setting, and 25 hours in a SNF setting would fulfill this requirement. 
  • An example of NOT acceptable PT exposure is: 90 hours in outpatient, and 10 hours in SNF(because of less than 25 hours in the second setting type.)
  • Similarly, 200 hours in outpatient at site "A", and 25 hours in outpatient at site "B" does NOT fulfill this requirement, because all hours are at one setting type only. A setting type is not the same as a site.

Only your prerequisite course grades will be used in calculating your GPA for admissions ranking.   Grades from any and all courses taken to fulfill each prerequisite will be considered.  If two courses were taken, the higher course grade only will be used.  If three courses were taken, the grades from each will be averaged.  If 4 courses were taken to complete one required pre-requisite, then the applicant will be disqualified.  A course is considered a “repeat” even if it is taken at a different university, or has a different course number from the same university.

Courses that need to have been completed within the last 5 years include Anatomy, Physiology, Kinesiology, and exercise physiology.

Advanced anatomy can be substituted for the basic anatomy prerequisite if it is to your advantage to do so. If advance anatomy is used for the basic anatomy prerequisite in the Additional Background Characteristics section of the supplemental application, no additional points can be earned on the supplemental application for taking advanced anatomy.

Additional information about program’s policy on foreign coursework 

Domestic students applying with transcripts from outside the U.S. should carefully review the webpage at http://www.csus.edu/gradstudies/international.htm and will need to submit their CSU Mentor Graduate Admissions application by 12/1/2012.

Foreign applicants need to have at least 8 units of prerequisite coursework completed prior to 10/15/2010 at a US accredited institution.  Applicants who do not possess a bachelor's degree from a post secondary institution where English is the principal language must demonstrate competence in English by scoring a minimum of 550 (213 if computer-based) on the Test of English as a Foreign Language (TOEFL). Foreign applicants must complete their observation hours in US physical therapy clinics.

Previous classes have included small percentages of out-of-state and international students (including Canadians). 

We do not give in-state students any preference in the admissions process.

PROGRAM DESCRIPTION

MISSION: The primary mission of the CSUS Department of Physical Therapy is to graduate knowledgeable, effective, adaptable and reflective physical therapist generalist practitioners who demonstrate ethical, responsible, professional behavior, are sensitive to cultural and psychosocial differences, use evidence derived from the clinical science research in physical therapy and related disciplines to inform independent judgments to meet patient needs, utilize critical and integrative thinking and problem-solving, and provide services that contribute to the optimal health and function of community residents.

CURRICULUM: The 36-month curriculum, including summers, encompasses didactic instruction, laboratory experience, and clinical education. The clinical component consists of thirty-six weeks of full-time, off-campus clinical internships .
